I always thought "A Clockwork Orange" (not THE clockwork orange, which I
think is very significant) referred to something kind of sick and
unnatural--an orange cannot be created from clockwork like a toy mouse.
It will cease to be an orange, just as Alex ceased to be Alex after his
brainwashing. And in the end, his nature could not be suppressed after
all. 

I think this is not too far off from a few of the responses that refer
to the phrase, "As mad as a clockwork orange." I had never heard that
before, but it really makes sense.
